Student Enrollment Procedure

Effective July 2006

Overview:

Kindergarten through 8th Grade: New students who are enrolling for the first time as either new residents or as kindergarteners should follow this procedure. For your convenience, enrollment is done by appointment only. Please contact the Registrar at 330-225-7731 to make an appointment.

High School: New students will enroll at Brunswick High School. Please contact the Guidance Office at 330-273-0494.

How to Enroll Students:

Only parents, legal custodians and/or legal guardians can register students in the District.

Obtain an enrollment packet at your child's school, at the Board of Education office, or forms can be downloaded from the Brunswick City Schools website by clicking on the links at the bottom of this page.

Call for an appointment.

Items needed at time of enrollment:

Student's Birth Certificate

b. Student's Social Security Number

c. Student's Immunization Records

d. Proof of Residency

*Acceptable proof of residency may include:

	1) lease agreement for rental property
	2) rent receipt
	3) utility bill (Gas, electric, water or sewer-within 30 days) We cannot accept telephone, cell phone or cable bills.
	4) voter registration card
	5) purchase agreement for a home
	6) property tax receipt from Medina County Auditor's Office
	7) deed for a home
	8) homeowner's insurance policy bill

e. Proof of Custody (if needed) Certified copy of Court Order or Court Appointed Guardianship.

f. Completed forms from the enrollment packet
