Preschool Medical Forms

Parents and guardians of preschool students are required to complete and submit various medical forms. Links to these forms can be found below.

Section 3301-37-08 of the Ohio Revised Code (ORC) requires Preschool programs in Ohio to obtain the following for all students: a report from a licensed physician affirming that the child is in suitable condition for enrollment in the program. The Physical Exam must have taken place within the 12 months prior to your registration date to be considered valid. 

If your child does not currently see a dentist, please indicate that on the bottom of the form. We strongly encourage you to take your child to a dentist.
